This program will focus on the Improving Medicare Post‐Acute Care Transformation Act (IMPACT) and the CMS hospital proposed changes to the discharge planning standards. This session will be beneficial for hospitals as well as Critical Access Hospitals (CAHs).

Sue Dill Calloway will focus on how to work with these proposed changes. Hospitals and CAHs should start thinking about how they will comply with these reforms, including the rewriting of the transfer form, the collection of five new data elements, and changes to the discharge evaluation form to collect five required standardized assessment elements. These elements will need to be included in all discharge instructions. She will also discuss CMS discharge requirements regarding medication reconciliation and patient medication side effects instructions. This will impact hospitals, home health agencies, inpatient rehabilitation, skilled nursing facilities, and long-term care hospitals.
