Learning Sessions and Rapid Cycle Improvement Project

Program Overview
This 3-part learning cohort features a multidisciplinary team from various organizations across Nebraska. The goal of the cohort is to review new and best-practices related to coagulation screening, prescribing, monitoring, transitions and safety. This work will help hospitals and clinics decrease patient harm and drive efficient and safe coagulation practices. Hospitals will hear from subject matter experts on keys to driving a safe coagulation program within their facilities.

Session #1 – Anti-Coagulation Stewardship, Transitions of Care, Resources
