This program will discuss the final surveyor worksheet for assessing compliance with the CMS hospital Conditions of Participation (CoPs) for discharge planning. Not only is discharge planning important to ensure compliance, but also reimbursement. This session will be beneficial for hospitals as well as Critical Access Hospitals (CAHs). These changes will also impact hospitals, home health agencies, inpatient rehabilitation, skilled nursing facilities, and long term care hospitals.

Our speaker will examine the IMPACT Act, discharge planning deficiencies and problematic areas. Sue will also cover physician dictation guidelines as well as various CMS standards for discharge planning. She will review new items noted in the CMS memo, including advisory practices for “blue boxes,” to improve patient outcomes, causes of preventable readmissions, inpatients vs. outpatients as well as detailed information about the discharge planning process.
