Unite Against Sepsis: Collaborating to Reduce Sepsis Mortality and Improve Care Coordination
Sepsis is the leading cause of death in U.S. hospitals. Sepsis is also the leading cause of readmissions to the hospital, with as many as 19% of people originally hospitalized with sepsis, re-hospitalized within 30 days. For these reasons, as well as many others, sepsis prevention, identification, and treatment remain a major healthcare priority. Fortunately, sepsis deaths can be prevented. According to the Sepsis Alliance, as many as 80% of sepsis deaths could be prevented with standardization and mobilization of tools and protocols that are already available.
Join Telligen QI Connect (HQIC and QIN-QIO programs) for a collaborative learning series aimed at supporting your facility in reducing sepsis mortality and improving care coordination. This learning collaborative has been developed using the newly released Centers for Disease Control and Prevention (CDC) Hospital Sepsis Program Core Elements. Featured content was originally developed for a hospital audience, however, we will also present opportunities for collaboration with post-acute care facilities. This series will include live presentations from subject matter experts, case study discussions, and guidance on using the Institute for Healthcare Improvement’s (IHI) Breakthrough Series model to improve your sepsis program.
